Parent Questionnaire

are your thoughts and/or feelings about contributing, along with your
child and her or his teacher, to your child’s report card?

important or beneficial do you think it is for you, as a parent, to
be involved in the report-card writing part of three-way conferences?

prepared do you feel for the report-card writing part of the conference?
What helps or hinders your feelings of preparedness?

could be changed or added to help you feel more prepared for discussing
your child’s progress and contributing to the report card?

are some barriers, if any (for example: lack of time, not feeling knowledgeable
enough, don’t like three-way conferences, etc.) which limit your
contributions?

do you think and/or feel about the teacher’s conduct during the
conference? (For example, not listening closely enough, not giving enough
direction, etc.)
