Parent Questionnaire

 

  1. What are your thoughts and/or feelings about contributing, along with your child and her or his teacher, to your child’s report card?

 

  1. How important or beneficial do you think it is for you, as a parent, to be involved in the report-card writing part of three-way conferences? What are your reasons?

 

  1. How prepared do you feel for the report-card writing part of the conference? What helps or hinders your feelings of preparedness?

 

  1. What could be changed or added to help you feel more prepared for discussing your child’s progress and contributing to the report card?

 

  1. What are some barriers, if any (for example: lack of time, not feeling knowledgeable enough, don’t like three-way conferences, etc.) which limit your contributions?

 

  1. What do you think and/or feel about the teacher’s conduct during the conference? (For example, not listening closely enough, not giving enough direction, etc.)

 

  1. What changes would you make to the three-way conferences that would increase your involvement and be more beneficial for your child?

Student Questionnaire

 

  1. What do you think about having your mom or dad help you write your report card?
  2. What do you think about you helping to write your report card?
  3. How do you feel about how much you contribute to writing your report card?
  4. What ideas do you have that could help both you, and your mom or dad add more information to your report card?
